Form End Notes:
Citizenship info for the form can come from an external warehouse table or from the KC Person Extended Attributes field of Citizenship Type [source determined by the parameter (PI_CITIZENSHIP_FROM_CUSTOM_DATA)]. Explanation: The NIH defines a clinical trial as a prospective biomedical or behavioral research study of human subjects that is designed to answer specific questions about biomedical or behavioral interventions (drugs, treatments, devices, or new ways of using known drugs, treatments, or devices). Explanation: An NIH-defined Phase III clinical trial is a broadly based prospective Phase III clinical investigation, usually involving several hundred or more human subjects, for the purpose of evaluating an experimental intervention in comparison with a standard or controlled intervention or comparing two or more existing treatments. Often the aim of such investigation is to provide evidence leading to a scientific basis for consideration of a change in health policy or standard of care. The definition includes pharmacologic, non-pharmacologic, and behavioral interventions given for disease prevention, prophylaxis, diagnosis, or therapy. Community trials and other population-based intervention trials are also included.

PHS 398 Training Budget V1.0

Data requirements: Users are required to answer this form-specific Questionnaire and input budget stipend details to fulfill all the form requirements.
Questionnaire: Questionnaire ID #3, version 1 supports this new NIH Training Budget form. (Question ID's 66 through 98) Versioning the Questions and Questionnaire is not recommended. The only allowable versioning of the Questions would be to alter the question language or answer length; the Questionnaire would then need to be versioned locally to include the latest question versions. Deleting or adding questions to this questionnaire will break the ability to support the s2s submission of this form.
New KC Maintenance Table: A shared maintenance document of Training Stipend Rate was added to support Stipend calculation from questionnaire entries. The Training Stipend Rate consists of Career level, Experience Level, Stipend Rate, and Effective Date. The effective date of a rate that fits most appropriately with the start/end dates of the proposal will determine which stipend will be used in the calculation and population of the form. These stipend rate columns should be maintained with the most relevant NIH published rates that occur on an annual fiscal year basis (once the NIH annual budget is approved by Congress).

Trainees: The number of Trainees in this form's categories (Undergraduate, Predoctoral, Postdoctoral, short term, full term, single degree, dual degree, non-degree seeking, degree seeking) requires utilizing the Questionnaire for the data to populate PHS 398 Training Budget form, as this information could not be easily captured from the KC Budget.
Stipends: Because the stipend rates are regulated by NIH, a shared maintenance table has been created using the current 2011 & 2012 NIH rates (NOT-OD-10-047/NOT-OD-11-067) to calculate and populate this form based on the entries in the Questionnaire. The exception is for 'Other' trainees, in which case the stipends and tuition are taken from the questionnaire; user should still input these expenses in their detail budget for internal records. The number of trainees for all remaining trainee categories must be input in the questionnaire; the stipend expense will be calculated and published to the form from stipend rate tables. User should also enter the stipend expense in their KC line item budget for internal record and a stipend budget line item equal to or greater than the calculated stipend amount from the questionnaire is required before the form can be printed/submitted.
Tuition: Users must enter tuition expense lines for each trainee category. To support tuition budgeting for this form six (6) parameters have been created to identify existing or created tuition cost elements. These budgeted tuition amounts for the Trainee categories for this form are published to the Trainee budget form.

If the selected opportunity only allows a three (3) years of support, when presented with the question "Are you requesting funds for Budget Period 4?" and "Are you requesting funds for Budget Period 5?", answer 'No' to these questions to leave those budgets blank. PHS Training Budget Creation Tips:

  • The Training Budget form cannot be print-previewed until stipend expense line items are entered in the KC budget to match the trainee numbers (using cost element 422315 'Stipends – Not MTDC'; found in Participant Support panel) and stipend levels are all answered in the Questionnaire. If the amount of stipends (422315) in the detailed budget is not equal to or greater than the amount of calculated stipends from the questionnaire you will receive validation errors preventing print/submit actions.
  • At print/preview, a KC validation error will alert the user of the budget deficit for the annual and cumulative requirements. The user only needs to enter the annual funds requested. How detailed the stipends are entered in the budget depends on your local campus practices.
    • A SINGLE line item for stipend expense in each budget period is sufficient to support the form.
    • Multiple appearances of the same cost element (one for each trainee type and stipend rate) will aggregate to validate the form. The detailed entry in the budget may prove to be easier to review and revise the number of trainees in each category.
  • CAUTION: Stipend funds in your KC budget in EXCESS of the Questionnaire calculated amounts will NOT cause an error and WILL appear in PHS398 Training Budget: Section B: Other Direct Costs, in the line "Total Direct Costs from the R&R Budget Form (if applicable)", and will be included in the total costs on both the Training Budget form AND the SF 424 (R&R) form.


Example:

  • Once the minimum stipend expense for each budget year has been entered in the KC Budget, the form can be previewed. Notice that the Tuition fields have no funds entered; Tuition expense MUST be entered in the budget, using the Cost Elements defined in the KC Parameters that support this form.



Example of Stipend in the KC budget that exceeds the calculated amount based on the answered Questionnaire. The excess amount appears in the Total Direct Costs from the R&R Budget Form line – the value $8,588. These funds will also publish on the SF 424 R&R form. Users should be careful to use the authorized NIH Stipend Rate for each category to coordinate with the Questionnaire calculated amounts.

Prior Form Versions

There are several methods to populate the Federal Identifier field to meet the varied sponsor requirements for this data. Please read your specific opportunity instructions and sponsor application preparation guides to understand what data and format should be provided for your application. END USER maintenance of the Sponsor Proposal No. field will trump any linked/system-filled method described later in this section as sponsor instructions and formatting vary so wildly.
Examples of some known usage of "federal identifiers" for s2s submissions:
NIH:
For a "New" Type of Application, leave the Sponsor Proposal No. field blank.
For a "New" Type Changed/Corrected Application, enter the Grants.gov Tracking Number of the previous application that you are correcting in the Sponsor Proposal No. field to fill the Federal Identifier field (Item 4) field. If you are unable to recall the Grants.gov tracking number, you can enter "N/A."
For a "Resubmission", "Renewal", or "Revision" Type of Application, enter the NIH IC and serial number of the previously assigned application/award number (e.g., CA987654) in the Sponsor Proposal No. field to fill the Federal Identifier field (Item 4).

  • Resubmissions: the IC/Serial number is in the prior KC Proposal record, in the Grants.gov panel, Submission Details tab, Agency Tracking ID field.
  • Renewals and Revisions: the IC/Serial number is part of the Sponsor Award Number and should be located in the KC Award. Otherwise, confirm this number at NIH eCommons.

For a "Resubmission", "Renewal", or "Revision" Changed/Corrected Application, enter (or retain the number from the previous submission attempt if it was correct) the IC and serial number of the previously assigned application/award number (e.g., CA987654) in the Sponsor Proposal No. field to fill the Federal Identifier field (Item 4)
DOE:
For a Renewals or Revisions to the Department of Energy, a DOE Project Identifier is often required. Follow the opportunity instructions for the data to insert in the Sponsor Proposal No. field in KC Proposals.
NOTES on Automatic Population of Federal ID will occur under the following conditions:

  • IF Federal Identifier is required THEN
  • IF the Proposal Type = Renewal, Continuation or Revision, AND the Sponsor Proposal No field is null, AND the parameter: FEDERAL_ID_COMES_FROM_CURRENT_AWARD parameter is set to 1 THEN, the system looks for a value in the Award No. field (from that Award record) to populate the Federal ID.

Please follow Proposal User Guide instructions for basic budgeting instructions. Cost Element mapping to Budget Categories in your local environment will impact where your expenses appear on these forms. This guide sheet will provide some tips to understand where expenses appear and how to reassign Budget categories at the proposal level to meet sponsor budget requirements. See the budget details section for detailed budget category instructions.


Base Salary field for Senior Persons
The Base Salary by Period table provides a mechanism to control the Base Salary values that appear on certain Grants.gov Budget forms (RR Budget 5 & 10 year, Fed/Non-Fed Budget 5 & 10 Year). Requirements for Use: Maintain an inflation rate factor in the parameter, but users may manually enter calculated values. Administrator role required to maintain the parameter DEFAULT_INFLATION_RATE_FOR_SALARY.
Using the Calculated Base Salary inflation tool:
The Project Personnel panel has a hyperlink that opens a light-box window where the fields for base salaries per period:

  • Click the "calculate" button to have the system multiply the Base Salary field by the centrally maintained rate.
  • Or manually enter the base values you wish to appear on the RR Budget form.

The number of budget periods available is determined by the number of budget periods established. No more than 10 budget period fields will be provided as the Grants.gov budget forms provide only 5 or 10 budget periods.



Tips for Senior/Key Persons:
> For the 5 year budget: A maximum of eight (8) Senior/Key persons can appear in full detail in Section A. The PI is always listed in the first line. If your budget has more than 8 senior persons and Addition Senior Key Persons attachment will be automatically generated and submitted with this form. . (These will appear in the Abstracts and Attachments/ Proposal Narratives section of the proposal.)
> For the 10 Year Budget form: Section A: Senior/Key personnel can support up to 100 persons. Addition Senior Key Persons attachment will be automatically generated and submitted with this form.
Senior/Key Budget Person Order:
The order in which proposal person names appear in the Key Personnel screen is the order in which they will be published on forms AND in the Grants.gov (S2S) RR Budget section A Senior/Key Personnel. The arrow/sort order tool allows the user to manipulate the sequence of these proposal persons to conform to sponsor instructions or to adjust the listing after synchronizing hierarchy proposals.To change the order, click on the arrow icon to move the proposal person up or down. The PI will always be listed first and cannot be moved. The last person listed will only have an up arrow; the first person listed will only have a down arrow; persons in the middle of the list will have both an up and a down arrow.


  • Persons maintained as the Investigator, Multi-PI, or Co-Investigator will always map to Key Person section, if applied to the budget (trumps budget category)
  • If your personnel cost element is NOT mapped to Senior Personnel but should be for a particular submission, change the Budget Category for this line item after you add the person to the budget person detail.



Other Personnel:
Other budget personnel will populate in this section, unless specifically mapped to the Stipend section.











Tuition is considered an "Other Direct Cost" by definition on this Budget form, and is grouped with other similar expenses in line 8.

H. Indirect Costs:
The indirect costs will be detailed by rate. If there are multiple applicable rates for the period, an indirect cost line will be populated for each rate.
Cognizant Federal Agency
The information (agency name, POC name and phone number) is populated from the proposal's Organization record, using the address book entry for the maintained cognizant auditor.
J. Fee: Generally, a fee is not allowed on a grant or cooperative agreement; we do support this field.

proposal development. This field is either manually entered or automatically populated if a grants.gov opportunity is attached to the proposal via the opportunity lookup on the grants.gov tab.
Technical notes:
Program title of the proposal (OSP$EPS_PROPOSAL.PROGRAM_ANNOUNCEMENT_TITLE) is populated from the opportunity title (OSP$S2S_OPPORTUNITY.OPPORTUNITY_TITLE). This should be the CFDA title, which is not available in KC. Department and Division names are optional elements in the schema. DAT file instruction for Department states "Enter the name of primary organizational department, service, laboratory or equivalent level within the organization which will undertake the assistance activity." Division should be "the name of primary organizational division, office or major subdivision which will undertake the assistance activity."

Box 8 - Primary Contract/Grants Administrator
Name and contact information of person to be contacted on matters involving this application comes from the organizational contact person. Person to be contacted on matters involving this application. This field is driven by the value of the parameter PROPOSAL_CONTACT_TYPE. The value of this parameter can be as follows:
• O (default) - OSP administrator for lead unit
• I- organization rolodex contact
• A - administrative officer from lead unit
• U - unit head of lead unit
• D - dean/VP of lead unit
• H - other individual to notify for lead unit
• a number (unit administrator type) - from osp$unit_administrators - if there is more
than one person, use the first
• C-FEL - fellowship coordinator - If the proposal is a Fellowship (activity code = 3 or 7), the person to be contacted will be the Fellowship coordinator (unit_administrator type = 3) for the top unit. To get the 'top unit', start at the lead unit for the proposal, and travel up the hierarchy to find the first unit with an organization. If there is no fellowship coordinator for the top unit, then person to be contacted will default to the OSP coordinator for the lead unit.
--NOTE TO MAPPING: It was decided KC will behave differently than COEUS from the above mentioned mapping. Instead KC will map the contact person default from Unit Administrator Code 6 'Grants.gov Proposal Contact' since this administrator type is limited to only one entry and was created specifically for this field. See KRACOEUS-3736.

NEW TOOL: User Attached Forms

KRACOEUS-6855 released in 5.2.1
This tool enables users to upload individual "unstitched" Adobe forms and/or any Optional forms "stitched" into a regular Adobe Opportunity to meet submission requirements when the target form(s) is not mapped for Coeus submission to Grants.gov. This solution means that the data is not entered into KC fields; these filled forms are treated more like uploaded narrative files and are submitted with the s2s opportunity to Grants.gov.
Things to know about using the attachment tool:
Where do I find the forms?
Unstitched forms are provided by Grants.gov. They can be retrieved at the Grants.gov web page http://www.grants.gov/web/grants/forms/
Forms listed with an asterisk (star) in the PDF column are unstitched and available for use with the upload tool.
Note: the comment at the top of the table – that the forms are "not submittable" – means that they are not intended to be submitted individually to Grants.gov. These special forms have been 'unstitched' to be included in our S2S submissions as part of a full submission that includes the other forms in the posted opportunity.

What about the posted Adobe Opportunities? Can I use those?
Yes, but you can only use the Optional forms in a packaged opportunity. If the form you require is listed in the Mandatory section, you will need to retrieve it as an unstitched form – Mandatory forms are not processed in the upload translation.

How do I fill out the form?
Unstitched forms do not have a validation tool on the page. If you are using an unstitched form, you may wish to also download your submission's Grants.gov opportunity to use as a reference. If you upload an incomplete unstitched form to the User Attached Forms tool, it will not translate, which means it did not validate and cannot be included in the Grants.gov submission until it validates. You will receive the grants.gov form schema –level error message. (The error message may not provide specific faults, but will confirm that the form is incomplete and does not meet the validation requirement.) You must update the form and repeat the upload.
If you are using an Optional form in an Adobe Opportunity, click in the box beside the form name to select it for inclusion. You may then click on the form name to navigate directly to it, or scroll down until you get to the form. Follow sponsor-specific instructions for your submission to complete the data requirements. The minimum form-level data requirements can be verified by using the "Check Package for Errors" button on the cover page. I you receive the "Complete" mark; the form has met the minimum validation. (This may not meet the sponsor's requirement.) You must provide at least the minimum requirement to successfully translate the form.
We do not have the ability to support Mandatory forms in an Adobe Opportunity, due to the "stitched" nature of data flow between forms. If the form is mandatory, you will need to locate and use the unstitched form.
When should I connect to the Grants.gov Opportunity?

  • As with any S2S connection to Grants.gov opportunities, if the all the mandatory forms are available, you can connect to the opportunity at any time.
  • If the forms you will be uploading using the User Attached Forms tool are Optional, you can connect to the opportunity either before or after you make the connection:
    • If you connect before uploading the forms with the tool, the Grants.gov Forms section will show those optional forms as "unavailable".
    • If you connect to the Grants.gov opportunity after uploading the forms with the tool, when you connect to the opportunity, the Grants.gov Forms will display as 'User Attached Form" and allow you click the box to include it in the submission.
  • If there are mandatory forms in the opportunity that are not available in KC, you will need to upload those unstitched forms first and then connect to the opportunity. We have added the names of the unavailable forms to the 'not eligible for KC transmission to Grants.gov" message window to alert users to the specific forms making the connection ineligible and the need to use the new tool to meet the requirement.




Notice at the bottom of the message, the Missing Mandatory Forms List has been added to inform the user of the specific missing forms.

  • If you connect to the Grants.gov opportunity PRIOR to uploading the forms, the optional forms will show as "Not Available" in the Forms subpanel.



  • If you connect to the Grants.gov opportunity AFTER uploading the forms, the forms will show "User Attached Forms" and the user can click to include them in the submission.



  • If there are Mandatory forms that are unavailable, you cannot connect to the opportunity until the List of Mandatory Forms is satisfied:

Step-by-step:

  • Navigate to the S2S tab and expand the User Attached Forms subpanel
  • In the Add row, enter some information about this upload in the Description field. Ex. the name of the target form, like "Planned Report" if you are uploading a single unstitched form.
  • Click Browse to open a search tool window; locate your unstitched form or Adobe opportunity with the target form completed. When you complete the search, the File Name field will display the location path of your upload,
  • Click the [add] button. If the target form(s) are complete, the Namespace field will update with the official form information, and the Form Name field will update from the form type name; the Form File Name will appear (name the user gave the file).
  • To remove an upload, click on the row to select it and then click the delete button.
  • To add another form, click the add button and repeat the steps noted above.
  • To view a form, click on the row to select it and the click the view form button.
  • Connect the proposal to the Grants.gov opportunity. The uploaded forms will show as 'User Attached Form" in the Description column.
